I am filling out my citation questionnaire...and I'm not confused but... I'm try to ascertain the thoroughness of filling it out. If on my ticket, I got cited three times for three different things...on the questionnaire should I list them in each offense box seperately even though they happened on the same day, and they were on the same ticket. Lets say I had foreign registration...I was in the HOV, and had front tinted windows..they were all issued by the same officer...same day and place...would I need to list each citaton seperately or all together in the offense box..? At this point no question is a dumb question and this is my first time doing this so I don't feel retarded.: Help?

What I don't get...is how people are viewing this topic...and not answering the question. Some of the people actually viewing the topic are past this step in the hiring process...all I'm asking for is a little..help.

Easy does it....I have yet to see any of these guys not answer a question for someone. Sorry I can't help but just give it a little time.

I'm sorry I'm not a lot of help..I asked him and he said that he doesn't know what the citations were for...Whathever they were moving violations or whatever..But he thinks that since it was one stop and it was one ticket, it should go in one box together.. I'd wait till one of the seasoned officers gives you a better reply..I tried for you! But i really think he was more concerned about what was for lunch than trying to help a brother out.. =/

Patience, dude.....give it a little more than 35 minutes before you get all puffed up. It's the middle of the day and a lot of the people who can answer your question are at work. There are probably people viewing the topic out of curiosity, but aren't able to answer the question (myself included, since it's been almost 25 years since I filled out my PHQ.) If I were in your shoes, I'd rather wait for a straight answer from someone who knows than to get a bunch of half-assed guesses from everybody who views the topic.

wannabe...
1. patience.
2. in the box where it asks for a description or offense, list all three issues.

i.e: (i don't remember the exact boxes on the from, but i know they were something like this)...

date
January, 2001

location
Los Angeles, CA

charge
HOV Violation
Front Tint
Invalid Registration

disposition
guilty (paid fine, removed tint)

it will be tight, since i know they don't give much space, but do your best.

If you can neatly list all violations in one line, then do as Processing suggested and list all offenses in one box. If if you need to use extra lines, go ahead -- but yes, do list all "counts".

does anyone know the website where you fill out the form online to make it look better, my recruiter told us about it but I can't find it?

The basic application ( 678 I think) is here: http://www.spb.ca.gov/Employment/stateapp.htm

I'm pretty sure you have to do the citation and arrest history one by hand. Someone correct me if I'm wrong.

also, unless they changed it, that form that you use from the web-site does not fill in you SS number on every page like it is supposed to so you have to go back and add it in.

Nothing states the form must be handwritten, so you should be okay.
