I am nowhere close to the PHS portion of the process at this time. But I do have some questions. Out of these twelve items on the list, I have questions about item number two (Birth Certificate) and item number five (Marriage Certificate).

I was not born in the United States and do not have birth certificate from my home country. Would an affidavit from the birthplace be sufficient? Secondly, my wife was born and raised in the United States. We got married overseas and have marriage certificate from there, would that be enough?

Thanks
GS

Marriage Certificate from overseas should be fine... I don't know how you'd get a local one if you weren't married here. As for the Birth Certificate, use the time now to see if you can get a certified copy from your home country. If absolutely impossible, advise your background investigator you've exhausted all means and go from there.

Thank you DW. I have already requested my cousin overseas to look into my Birth Certificate. I will exhaust all the resources to get it and if impossible, then I will inform the background investigator of my situation. I will also be checking with the County office here to see what is available in regards to the Marriage Certificate.
