EYE SURGERY

I had lasik surgery on both of my eyes on July 1, 2004. (almost 2 years ago).
Am i still required to turn in 1 years worth of eye checkups.
Does anyone know the number to the doctor in Sacramento (i lost the paper where i had his number)? THANKS FOR ANY INFO.

I've gone through this process.

Yes, you are still required to turn in one years worth of eye check-ups. No less than 3 months apart. You'll most likely have to take another eye exam once the SPB Medical Officer has reviewed your stability.
