Im taking my physcial and psych test this weekend.
I assumed that I only have to turn in my "CADET ARREST AND CITATION QUESTIONNAIRE"

Is this right?

Do I turn in Insurance policy and School transcripts at my 1st interview w/ my background investigator?

You will turn in your CHP 446F, Cadet Arrest and Citation Questionnaire as well as your STD 678 Application.

You are correct the other information you described will be collected during your initial interview.
