View Full Version : Personal History Statement, need a quick answer!

When i turn in my personal history statement what should be in the packet? obviously the statement and the notarized release of info. but am i missing anything. i don't want to turn it in and not have it perfect

If I remember correctly, you'll submit the PHS by itself anfd have the ancillary documents available for review by the time your initial interview is scheduled.

You need to turn in the actual PHS packet, including page 23 of 23, which has 3 boxes that tell you to read and sign. You only read and sign the first box, as you will sign the other two during your BI. You also must include the Authorozation to Release Information sheet (I think there is two of them, one of which must be notorized. Lastly there is an All Purpose Acknowledgment form that must be notorized and returned. Also, if you were or are in the military, you must fill out the CHP 446C form.

Lastly, go through your packet a million times and make sure every single question that applies to you is answered, and any that do not apply, make sure you write "DNA" in the box. Right "DNA for EVERY box that doesn't apply.

I made a copy of my PHS, just for my own records. It is also good to have your own copy so between now and your Initial Interview, you can write down any changes that have been made since you submitted it. WHen you have your Intial Interview, your BI will ask you if anything has changed, and its better to have all the info written down so yo don't miss anything.

I typed out all of my changes and used that during my Intial to speed up things. And once you think you have checked all of your forms , check again. This is your career, and you don't want to mess anything up. Good luck, and make sure you let everyone know your process.

I submitted my notarized forms w/ my PHS, but it may not make a difference. My BI didn't say anything to me about it. If you want to make sure, just call the office in which you are sending the packet, and I am sure they can tell you for sure.

Very good advice. Check, recheck and recheck again. Then after you have recheck it, have someone else check it. Its amazing what our brain will fill in for us.

i am lost on the all purpose acknowledgment form, i only can find(or thought) that the release info. form is the only notarrized form. i check other documents and find no reference to this one. please let me know if i am missing something or if i have it right. THANK you for the responses so far

also can i tear the military records paper and the waiver(to hold of the BI for 60 days) and not send those in. they seem to have no purpose to the packet since i am not military and want my BI to start now:smile:

The Authorozation form is CHP 446B. It should have been with your PHS. Im not sure about the military forms. You may want to send them in just in case.

I just submitted my completed PHS, along with the notarized form and my signed commitment to hire form. I believe the Authorization to Release Information sheet is page 3 of the PHS. You must submit every single original form, even if it does not apply to you.

Yes, I have already turned my in. I just forgot the wording of one questiong that has made my lil brain tick lately.
