For the PHS (personal history statement) / background investigation, I need the social security 10 year work history record. Does anybody know what the form number is for the 10yr work history?

I had to get a form from the Social Security Office and mail it off requesting my report of earnings. I think it could take a while too.

Go to the Social Security website. It think it is www.ssa.gov (http://www.ssa.gov) and I think you can order a statement of earnings on-line. Not sure how long it takes but I would bank on a least a couple of weeks.

I went to my local SSA office and told them I needed a summary of the last 10 years for a backgrounds with law enforcement. They printed it out on the spot and gave it to me for free. I just had to wait about 20 minutes until it was my turn. Bring your social security card and passport. Picture ID wasn't good enough... at least in San Diego. My background investigator accepted it.

I originally thought I had to fill out the form and pay. I filled it out and had my checkbook out but apparently that's not the case.

hmmm maybe because you live in a bigger city, i tried to do that they made me fill out paper work and send it off. interesting

Same, I stopped by, and they told me to send it in, and i live in the Bay Area.
now i just hope all my stuff arrives in a timely manner, and it doesn't get lost

Hmm must be a San Diego thing. I walked in, showed the lady my DL and SS card and she handed me the paper work. No forms to fill out.
