I beleive it needs to be notarized by a Public Notary! Most banks have a Public Notary who notarizes free for members. Otherwise look in phonebook for Public Notary and you can pay a few bucks and have em done.

Can anyone clarify the matter of the Authorization to Release Information form and how or by who it needs to be notarized.

I did as RAT suggested and found a Notary Public in the yellow pages. It took 5 minutes for him to sign everything and document the event. I asked how much I owed and he said, "Don't worry about it. I really respect people in law enforcement and those who are willing to sign up for such a respectable career."
