Im not sure but i thought it was pretty funny how the PT Officer said, "there goes your career, you might as well just leave now". Just one lesson not to be late!

that was pretty good. I have already started to fill out my packet and dang, its gonna take some time to dig up some information.

Also is it a good idea to bring it to my QAP or should i wait? All handwritten documents produced by the Department will be in block capital letters. While it is not required on the PHS, printing neatly in block lettering is a good idea. Whatever you do, make sure it is neat and has a professional appearance.
You should have been given specific instructions by your Division regarding when to present your PHS. If you were not told to bring it to the QAP, don't. Most Divisions should be collecting the PHS during the weekend of the PAT/Psych, so you won't even have your PHS to present at the QAP.

Inside the PHS packet it asks for 5 references, they cant be past or current employers, co-workers, relatives, and no school teachers. So pretty much that leaves me with few references. I only stay in contact with one high school friend, but most of my other friends that know me or know of my qualifications are people that i work with, or my college criminal justice teachers. Hmmm any advice, any one else run into this?
Hmmm any advice, any one else run into this?Yes, several. Search for "references." Romantic partners? Families thereof? Family friends?

We made some changes on the last test cycle and all Divisions should be pasing out the PHS during the weekend of the PAT/Psych written. For the time being, GGD is handing them out at the Psych Written. You will then have two weeks to complete the document and turn it into GGD by dropping it off or mailing it in. We will give you a specific date it must be completed. I highly recommend you begin working on the document as soon as possible. Don't put it off until a night or two before it's due. You will find yourself turning in an incomplete unprofessional document. If you really want this career, you will take the time to do a thorough job on the PHS.

You'd think I would remember that, wouldn't you? I'm not sure what I was thinking when I wrote that. We need to stop changing things around, I'm easily confused as it is. And don't get me started on the "new" way to perform CPR (new for which year???).

I know how that guy may have felt. I was late to the PAT because the lovely Beale AFB had their lovely gates closed and I didnt know it... It took me almost an hour to get off of that lovely base....and that made me 30 min late to the PAT....
Once I got there, here come the sharks.....firing questions at me from left to right. There was nothing I could do, I didnt give any excuses, took my butt chewing and moved on!
Lesson learned....dont stay on the AFB anymore, and just sleep at the Mickey D's down the street....lol

I had a hard time with that one too. I lead a quiet life and most of my friends are past or present co-workers. I work at a feed store and used a couple of regular customers I have come to know over the six years I worked there. I also used some neighbors that I know. I live in a fairly rural area and what few neighbors I have are good ones!

Hey folks,
The CHP will give you all the answers you need, when you need them. If you haven't heard about the PHS, yet, then it will not be required of you. When you do hear about it, not only will they hand you the necessary paperwork, they will give you explicit instructions about what they just handed you. On top of that, you'll probably have another sheet that reiterates all the instructions they just verbalized. Don't stress about the things you don't have to.

+1
Here's the key...'Do what you are instructed (read - 'Told'), When you are instructed', nothing more....nothing less. There is nothing wrong with a little preparation (ie...getting your old address from the 10th grade, determining the date you got that speeding ticket, etc...), but as far as paperwork goes, when the Department needs something from you....rest assured, you will receive that paperwork.
Read.....Freaking Relax!
