I'm on the "immediate family" portion of the PHS and there are entries for mother/father in-law. My in-laws are all in Europe and do not speak English at all. The telephone number and address fields are also incompatible with non-U.S. information. Should I enter their information anyway?

Yes, if they're asking for it, they want it. The more information you can give them the better. They will choose if they need it or not.

Maybe we have a different PHS packet? I never saw any entry for inlaws. I have copies of my PHS here. What page and letter are you reffering to?

I found this PHS on the Internet. It might be the wrong one, but I thought I'd use it to get a head start. I just took the GED exam on Monday and Tuesday, but I only have the results for 4 of the 5 exams. I have to wait until I know I've passed the 5th exam before I drop my CHP application in the mail.

You probably found a general POST PHS. We use our own so don't get too comfortable with the one you have.

I'd keep a list of those relatives contact information for when you have your interview with your BI. They will gather that stuff there.

My sister is Australian' a real Tasmanian Devil! I gave her address & phone number. Phone number isn't our (xxx)xxx-xxxx style; I filled in how we dial her direct (country code, area code, phone number). I also offered her email address.

I thought the CHP PHS was actually easier to deal with than the standard POST one that is available for download.

Just a suggestion to avoid confusing anyone else or causing any unneccessary questions,,, wait until you get the "CHP" PHS. If you want, get the information reuqested on the POST PHS then transfer any information requested to the CHP PHS and forget about the information not requested.
